SERVICES

At Ogden City Risk Management we Administer the City's Insurance Programs including all Claims. We have the Responsibilities of Processing the Following Services:

  • Workers' Compensation Claims
  • Public Liability Claims
  • Property / Auto Damage
  • 3rd Party Insurance Claims
  • Emergency Management Services
  • Safety Services

Workers' Compensation

The City's Workers' Compensation Plan is self-insured and self-administered in accordance with the Utah Labor Code. Staff provides information and assistance to injured employees and other defined customers including all City departments. Staff further investigates, determines and delivers appropriate benefits including: all medical and salary continuation benefits, death benefits to surviving dependents, disability benefits, and coordination of vocational rehabilitation for disabled employees in compliance with state and federal mandates.

Public Liability

The Public Liability administers a self-insured program for public liability (tort) risks, which is supplemented by purchased excess liability insurance. The division supports: investigation and settlement of all claims arising from the City's operation as a municipality; services to citizens and special event promoters to ensure the public's interests are protected with appropriate insurance; selection and purchase of all risk and excess liability insurance; and investigation and recovery of damages to City assets caused by individuals and corporate entities.

The following listing reflects examples of the types of claims that can be filed against the City:

  • Automobile accidents
  • Water main flooding
  • Sewer main flooding
  • Tow away
  • Trip and fall

Ogden City Liability Claim Form Procedures

  • How to File Claim

Any claim against the City must be made in writing using the Claim Against the City of Ogden Form and be completed in accordance with Utah Government Codes. Most claims must be presented to the Risk Management Department no later that one (1) year after the date of the incident or event. The claim form must be accurately completed, printed, signed by the claimant and mailed or personally presented in hard copy format to: Ogden City Records Office, 2549 Washington Blvd., Suite 201, Ogden, UT 84401  All claim forms must be notorized. Please allow 45 days for the Risk Management Department to process and investigate your claim. Please be advised that the City is prohibited from providing legal advice. Claimants should consult an attorney as necessary.

After the claim has been received by our office:

  1. An investigation will be conducted. Claims are usually processed within 45 days after receipt by our office. When filling out the form, please be as accurate as possible to expedite the investigation process.

  2. Provide written justification for the amount of loss indicated on the form:
    a.

    Property damage claims require an estimate and photographs of the damage.

    b.

    Personal injury claims need to be accompanied by unaltered copies of bills from treating physician(s) or other expenses incurred from the injury sustained.

Information requested by the Public Liability Division does not guarantee that your claim has been accepted for payment nor does it mean that the City accepts liability. Any information provided will be evaluated as part of the investigative process.

Safety & Emergency Management

The goals of the Safety & Emergency Management Services are to prevent losses to the City and to provide safe public facilities and employee work areas. The centralized staff of this program assists and works with line management in all City departments in an effort to reduce injuries and illnesses. They coordinate and conduct safety training, inspect work places, investigate and monitor employee injury and accident reports and work with City departments to develop safety and environmental health procedures.
